As the parent/legal guardian of the above player, I agree that in the event my child suffers sudden illness, accident, or injury and neither parents nor guardians can be contacted, I give permission for any emergency treatment that is deemed necessary by a licensed Medical professional, and agree to be responsible financially for the reasonable cost of the medical assistance and/or treatment. Recognizing the possibility of physical injury associated with soccer and in consideration for the ECHS Strikers accepting the registrant for its soccer programs and activities. Eligibility

Players shall be homeschooling students with the majority of their education managed by their parents or guardians. Age requirements for teams will match those required for the state home school soccer league. Elementary – 11U Players shall not turn 12 before the last day of the state tournament. Junior High – 14U Players shall not turn 15 before the last day of the state tournament. Varsity and JV – 19U Players shall not turn 20 before the last day of the nationals tournament. *My player is eligible based on the above statementMy player is not eligible based on the above statement and needs to be considered for an exception

Statement Of Faith

ECHS Strikers is a Christian organization and affirms the following Statement of Faith. Scriptures: God reveals Himself to us through the Bible, His only inspired Word, and it is completely accurate, reliable, and authoritative. The Godhead: God is the one and only true God, who exists eternally in three distinct yet completely united persons: Father, Son, and Holy Spirit. God is complete, holy, righteous, all-powerful, all-knowing, and loving. He is working to bring about His will for creation. People: God created human beings, male and female, in His image. All human life is sacred and of inestimable value in every stage or condition from conception until death. Salvation: All people have sinned and fall short of God’s glory, therefore all people are separated from Him and deserve to die. In love, God sent Jesus Christ, God incarnate, to earth to pay the price for our sin. Jesus was born of a virgin and lived a perfect, sinless life. He died on the cross as the perfect sacrifice for sin, rose bodily from the dead, and ascended to heaven where He still lives today. All who believe in His sacrifice and accept Him as Savior and Lord are adopted into His family and will live forever with Him in Heaven. The Return of Jesus: One day Jesus will return to earth to resurrect the dead. He will gather believers to Himself and everlasting life and judge unbelievers to punishment and everlasting separation from Himself. Family, Marriage, and Sexuality: God has created each person as either male or female. Human sexuality is a gift and a stewardship from God that cannot be altered. To reject one’s gender is to reject God’s image in oneself. Marriage is instituted by God as a lifelong covenant between one man and woman for the purpose of representing the relationship of Christ with His church, for human companionship, and for bringing children into the world. Sexual behavior is only acceptable within marriage. Children are a heritage from the Lord to be trained for His purpose.
